In effort to enlarge a small log cabin quilt my daughter had started, I sewed a second set of log cabin squares to add to existing quilt. The new squares ended up slightly larger so nothing matched :( . Ended up taking them apart and re-sewing.

I have made the mistakes you have stated but the one that always irritates me the most is when I need to rip a seam out, half way thru I will start pulling and end up ripping the material.

My biggest mishap was a Bible case. I measured and measure and cut and tried to do everything just right. I thought I had measured correctly when the zipper was installed. Unfortunately, it was slightly off. Not noticeable by the eye, just when you put the Bible inside and zippered it up.

My worst mistake was trying to make a USB/Chapstick holder for my daughter on my embroidery machine. The first time I put the material on the back of the hoop so that the wrong side was showing on the front of the design. The second attempt the back material shifted so one side wasn’t attached. The third time I took the hoop off to add the material to the back (this time using a spray adhesive) and everything hopped out of the hoop. On the fourth try I finally got it right. Something that should have taken less than an hour took more than two and left me wanting to pull my hair out! I’ve never been so glad to finish anything.

One of my biggest sewing mistakes happened when I was making a lion costume for my grandson when he was 2. I cut out 2 fronts instead of a back and a front. Luckily, I had purchased a little extra and I was able to angle the back in on the piece that was left. Not on the grain, but it worked out ok!
